ETS Graduate Student Authorization Form
Graduate students at the College of 黑料大事 must first complete a Graduate Student Authorization Form before taking delivery of equipment. This form must be filled out and signed by the graduate student's advisor or professor. The form is valid only for the remainder of the semester to which it is assigned.
